Information Practices Act Privacy Notice: The State of California Information Practices Act of 1977 (effective July 1, 1978) requires the University to provide the following information to individuals who are asked to supply information about themselves. The principal purpose for requesting information on this form is to provide information for appointment to UCD Administrative Advisory Committees. UC Davis Policy and Procedure Manual Section 200-15 authorizes maintenance of this information. Furnishing all information requested on this form is voluntary. Information furnished may be used by various University departments in identifying persons interested in serving on Administrative Advisory Committees and will be transmitted to the State and Federal governments if required by law. Individuals have the right of access to this record as it pertains to themselves. The official responsible for maintaining the information contained on this form is the Chancellor of the University of California, Davis, Offices of the Chancellor and Provost.
